TitleBubble inside bubble: Nested perfluorodecalin within a silicone oil bubble in the anterior chamber
PurposeTo describe a rare postoperative finding of a perfluorodecalin (PFD) bubble encapsulated within a silicone oil (SO) bubble in the anterior chamber following retinal detachment repair.
MethodsWe report a case of a 59-year-old male with chronic total retinal detachment in one eye, associated with multiple retinal tears and extensive choroidal and ciliary body detachment. Pars plana vitrectomy was performed using perfluorodecalin intraoperatively, followed by silicone oil tamponade. The eye was left aphakic at the end of surgery. A focused review of the literature on the physical properties, indications, and complications of PFD and SO was also conducted via PubMed to support the interpretation of clinical findings.
ResultsSlit-lamp examination in the early postoperative period revealed a PFD bubble fully encapsulated within an SO bubble in the anterior chamber. This configuration illustrates the distinct physical properties of the two agents: SO’s buoyancy and PFD’s high density. Retained intraocular materials can lead to complications including corneal endothelial loss, keratopathy, or elevated intraocular pressure.
ConclusionThis case highlights the importance of understanding the behavior of intraocular surgical agents postoperatively, especially in aphakic eyes, and the potential clinical consequences of retained substances.
