TitleWhen Appearances Deceive: An Atypical Conjunctival Lesion Resembling Sebaceous Carcinoma
MethodsDescription of a case presenting with an atypical conjunctival lesion, for which the definitive diagnosis was established through histopathological examination of an excisional biopsy.
ResultsA 57-year-old man was referred for evaluation of a conjunctival lesion in the left eye, clinically mimicking a sebaceous carcinoma with pagetoid spread. At the age of 21, he had undergone multiple interventions for recurrent pterygia; however, no operative reports could be retrieved. Biomicroscopy revealed a vascularized conjunctival lesion, with sharply delineated gland-like structures. Excisional biopsy and surface reconstruction using a conjunctival autograft were performed. Histopathology demonstrated the presence of Fordyce nodules, which are ectopic sebaceous glands that can be distributed along oral and genital mucosa. This finding confirmed our hypothesis that the conjunctival lesion originated from a buccal mucosal autograft. Postoperatively, our patient was highly satisfied with the aesthetic outcome.
ConclusionThis case highlights an atypical conjunctival lesion mimicking sebaceous carcinoma, which was ultimately identified as ectopic sebaceous glands (so-called Fordyce granules) originating from a previous used buccal mucosal autograft. It underscoresthe imperative role of surgical excision or biopsy with histopathological evaluation in cases of diagnostic uncertainty.
