Title A Sphenoidale cyst Presenting Headache and unilateral Papilledema grade 5 of Frisen grades.
PurposePapilledema is optic disc swelling caused by increased intracranial pressure . This case report presents a case of papilledema secondary to a sphenoidal cyst.
MethodsA 79-year-old man with diabetic retinopathy OD, hypercholesterolemia, and hypertension presented to an ophthalmological private practice with acute vision loss, headache, and unilateral papilledema lasting several months. He was subsequently referred to a hospital where he was initially diagnosed with Anterior Ischemic Optic Neuropathy (AION). However, an MRI of the sinus later revealed a sphenoidal cyst. The patient’s condition deteriorated, prompting admission to another hospital where he received augmentin IV. After this treatment, there was an improvement in the papilledema and his overall health, but the vision loss persists.
ResultsThis case highlights the role of disrupted axonal transport in retinal cells, which can result from factors such as ischemia, inflammation, or increased pressure. In this case, there was no prominent increase in infection and inflammatory parameters visible. This fact is highly suggesting an infection, especially in old diabetics.

ConclusionFor the elderly with high multi-factor risk, the absence of inflammatory symptoms and signs are not representative. It can lead to misdiagnosis and mistreatment. This case highlights the unusual presentation of a sphenoidal cyst with papilledema. Early neuroimaging can lead to an adequate diagnosis and treatment.
