Title | Case report advancing wavelike epitheliopathy: to cut or not to cut? |
Purpose | The purpose is to report the successful clinical outcome of a case of bilateral advancing wavelike epitheliopathy in which one eye was treated conservatively, and the other was treated surgically. |
Methods | This study is a retrospective case report. |
Results | A 26-year-old woman was referred to our clinic complaining of irritation and progressive decrease in vision in both eyes. She had no ophthalmic history except for bilateral contact lens (CL) wear. Slit lamp examination revealed an epitheliopathy covering the entire cornea with superior limbal vascular ingrowth and dot-shaped fluorescent staining in both eyes. Based on the clinical presentation and history of CL wear, a diagnosis of advancing wavelike epitheliopathy was presumed. Epithelial debridement with silver nitrate 1% application was performed in one eye, along with medical treatment with cyclosporine 0.1% in the postoperative period. The other eye was treated with cyclosporine 0.1% drops only. In the six-month follow-up period, vision was restored in both eyes, and slit lamp examination showed no more abnormalities. |
Conclusion | Advancing wave like epitheliopathy is a possible cause of epitheliopathy and decreased vision among CL wearers. Successful management options include the use of cyclosporine drops or surgical intervention. Our case illustrates comparable outcomes in the eye treated with epithelial debridement with topical silver nitrate 1% application and cyclosporine 0.1% drops and the eye treated solely with cyclosporine 0.1% drops. |
