Title	A not-so-innocent pupil-sparing oculomotor nerve palsy

Purpose

A 92-year-old woman with an unremarkable medical history presented with acute onset diplopia attributed to an incomplete pupil-sparing oculomotor nerve palsy. Her blood sample, including assessments of blood sugar and cholesterol levels, yielded reassuring results. Over a span of 2 months, her condition progressed to a complete oculomotor nerve palsy with pupil involvement. Concurrently, proptosis developed, and the optic nerve became affected, leading to a significant loss of vision. Coupled with this, there was palsy of the abducens and trochlear nerves, culminating in complete ophthalmoplegia. She also developed dysesthesia symptoms in the area of the trigeminal nerve (N. V1, V2, and V3).
Magnetic resonance imaging of the brain revealed the presence of a left skull base tumor with infiltration of the cavernous sinus, trigeminal nerve, Gasserian ganglion in Meckel’s cave, and the extraocular muscle bellies. Biopsy of rectus superior, rectus medialis and levator palpebrae superioris muscles confirmed a diagnosis of diffuse large B-cell lymphoma. Subsequently, she underwent radiation therapy (10 fractions of 3 Gray each) which led to clinical and radiological improvement.
