Title	DMEK versus DSAEK: Which corneal transplantation technique is superior?

Purpose	New corneal transplantation techniques are on the rise, but which technique is the best and can be used as a standard? In this study we compare the clinical outcomes of DSAEK, which is currently the most widely used and DMEK, a newer but more difficult technique which is said to give better visual results.
Methods	This literature review is based on 17 articles found by scanning different databases and taking into account selection criteria and the quality of the studies.
Results	Several parameters were compared between DMEK and both DSAEK and thin-DSAEK: best corrected visual acuity, endothelial cell loss (ECL), transplant rejection, iatrogenic primary transplant failure (IPTF), rebubbling and patient satisfaction. DMEK showed significantly better and faster visual results than DSAEK in all studies, and in half of the studies compared with thin-DSAEK. In 2/5 studies for DSAEK and 1/5 studies for thin-DSAEK there was 1 case of rejection and none in the DMEK group. ECL and IPTF were the same for all groups. The need for rebubbling was higher for DMEK compared to DSAEK (in 6/7 studies) and compared to thin-DSAEK (in 4/6 studies). In half of the studies, satisfaction was higher after DMEK than DSAEK (the other half was not significantly different) and was not significantly different comparing DMEK to thin-DSAEK. In all studies DMEK was preferred over (thin-)DSAEK.
Conclusion	Compared with (thin-)DSAEK, DMEK gives better and faster visual results. No major differences were seen in complications except for the number of graft detachments. Compared with (thin-)DSAEK the need for rebubbling was higher in DMEK. DMEK could be a better corneal transplantation technique if there is sufficient experience. However further research is needed.
