Title | A curious case of recurrent transient diplopia |
Purpose | Not many diseases are known to cause recurrent episodes of cranial nerve palsy. We discuss such an uncommon case involving multiple ocular cranial nerves over a period of two years. |
Methods | A 37-year-old man presented with 4 episodes of diplopia caused by different ocular cranial nerve palsies: left trochlear nerve (April 2021), right trochlear nerve (May 2022), left abducens nerve (October 2022), inferior branch of the right oculomotor nerve (March 2023). |
Results | Work-up including full blood analysis, lumbar puncture, single nerve fiber EMG, MRI brain, CT thorax and PET scan did not reveal any underlying etiology. All episodes resolved spontaneously. A trial with oral steroids did not result in improvement during the last two episodes. |
Conclusion | Literature research showed a very similar case, in which the MRI scan showed changes in the cavernous sinus only after almost 30 years. Despite not fulfilling all criteria, Tolosa-Hunt syndrome should be considered in case of recurrent cranial nerve palsies. Another differential diagnosis to be considered in our case is the exclusionary diagnosis of idiopathic recurrent polyneuritis cranialis. |
