TitleSurgical reconstruction using corneal patch graft after limbal lipodermoid excision
PurposeWe present the surgical reconstruction of 3 cases using corneal patch graft after limbal lipodermoid excision.
MethodsThree patients were surgically treated for limbal lipodermoid.
ResultsThree female patients (ages 4y, 18y and 8y) were diagnosed with limbal lipodermoids, causing both visual and esthetic complaints. All tumors were removed using lamellar corneoscleral dissection followed by anterior lamellar corneal patch graft transplantation. Pathology confirmed the diagnosis of benign lipodermoid with no sign of malignancy in all cases. All patients had a normal postoperative recovery, including a clear corneal graft, absence of inflammation and complete epithelial closure. Corneal astigmatism was reduced (from 2.75D to 0.75D , case 1) or remained stable (1.75D, case 2). Preoperatively, profound amblyopia was already present in case 3 and surgery was performed purely for aesthetic reasons. No lipodermoid recurrence was noticed during follow up.
ConclusionEpibulbar dermoids are the most common episcleral choristomas involving the globe in children. Although most lipodermoids are harmless, many still require surgical removal. A variety of surgical techniques has been described in the literature. Depth, size, and site of such lesions are critical factors. This case series illustrates excellent anatomical and visual outcomes after surgical resection combined with anterior lamellar corneal patch graft transplantation up to 3 years postoperatively.
