TitleStudy of risk factors for cystoid macular edema after epi-macular membrane surgery
PurposeEpi-macular membranes (EMM) can cause cystoid macular edema (CME). Some patients develop CME after surgical removal of an EMM. Paradoxically, both EMM and their surgical treatment are potentially responsible for CME. Our objective is to investigate the risk factors influencing the development and/or the persistence of a CME after an EMM peeling.
MethodsThis retrospective study included patients vitrectomized at Erasme Hospital for an EMM peeling over a period of 7.5 years. We compared group 1, defined by the presence of cysts and increased central macular thickness (CMT) on postoperative OCT, with group 2, defined by no cysts and decreased CMT postoperatively. A univariate binary logistic regression was performed to assess the association of the different factors with the occurrence of CME postoperatively.
Results7% of our sample presented a post-peeling CME. The mean age was significantly younger in group 1 (67.00±10.32 years) than in group 2 (72.10±6.84 years). In addition, 20% of patients in group 1 were treated with NSAID eye drops preoperatively and 13.33% with H1 antihistamines demonstrating a statistically significant association with these two treatments in our sample.
ConclusionOur study shows statistically significant risk factors for the development of post-peeling CME such as the young age of the patient and the use of NSAID eye drops and H1-antihistamines. A preoperative inflammatory field could be a confounding factor of the association with the NSAID eye drops. H1-antihistamines can disrupt the permeability of the blood-retinal barrier and the younger age of the patient causes a greater inflammatory response. A larger sample of patients should be studied in a prospective way to confirm our results.
