TitleOptical Coherence Tomograpy in Cerebral Amyloidosis

PurposeTo compare structural and vascular OCT parameters between cognitive intact (Control) and mild cognitive impairment patients (MCI) with (Aβ+) or without (Aβ-) cerebral amyloid deposition.
MethodsRetinal nerve fiber layer thickness (RNFL), ganglion cell complex (GCC) and parafoveal vessel density were measured using (angio-)OCT in Controls and MCI patients. Differences between subgroups were assessed by t-test. Diagnostic performance in determining amyloid status was evaluated using receiver operating characteristic (ROC) analysis.
Results35 Controls (16 Aβ+, 19 Aβ-) and 28 MCI patients (8 Aβ+, 20 Aβ-) were included. In the Aβ+ MCI group, RNFL and GCC showed a significant attenuation compared to the Aβ+ Control group (p=0.027 and p=0.006 respectively). Interestingly, a similar decrease was not observed between the Aβ- groups (p>0.05). Within the MCI group, inferior RNFL was significantly thinner in the Aβ+ group compared to the Aβ- group (p=0.005), with an area under the ROC curve for predicting amyloid status of 0.79 (p=0.018). No differences were observed in parafoveal vessel density between subgroups (P>0.05).
ConclusionThis is the first study to investigate retinal OCT parameters in MCI taking cerebral amyloid status into account. In our study the attenuation in RNFL and GCC in MCI appears to be confined to Aβ+ patients. These findings provide further support for the role of OCT in the diagnosis of Alzheimer’s Disease
