TitreEfficacy and Safety of endocyclophotocoagulation for Intraocular Pressure Reduction in glaucoma patients: A nonrandomized prospective pilot study on 13 eyes
ButThis pilot-study aims to assess the efficacy of endocyclophotocoagulation (ECP) in intraocular pressure (IOP) reduction and to evaluate its safety profile in terms of adverse effects, particularly intraocular inflammation.
MéthodesThirteen eyes of nine patients were included in this study. All patients were eligible to undergo an IOP-decreasing operation. We performed an endocyclophotocoagulation with EndoOptiks E2 810 nm diode laser. The energy was regulated intraoperatively based on the coagulation or audible pop of the ciliary body. A mean total power of 19 W was used. The ciliary body was treated on 270° through a corneal incision of 2.2 mm. The IOP was measured before the operation, and one week, one month, and three months afterwards.
Additionally signs of intraocular inflammation were closely monitored post-procedure.
RésultatsThe mean preoperative IOP was 23,4 mmHg and the mean postoperative IOP at one week was 15 mmHg. At one month, the mean IOP reduction was 8,1 mmHg.
Concerning the inflammation, at the one week postoperative control, we observed rare cells in the anterior chamber of six eyes, 0.5x cells in four eyes, 1x cells in one eye and 3x cells in one eye. One patient (with the highest delivered energy) developed an Irvine-Gass syndrome.
ConclusionEndocyclophotocoagulation has emerged as a mini-invasive potential treatment modality for reducing IOP in patients with glaucoma. The results indicated a notable reduction in IOP and a relatively low intraocular inflammation following ECP treatment, implying a favorable safety profile regarding immediate post-procedural complications. The procedure has a short learning curve and could be added and combined with phacoemulsification and other MIGS.
