TitreMacular neovascularization in chorioretinal coloboma: A case report of a 17-year-old patient
ButThe purpose of this report is to shed light on a case of a 17-year-old patient with bilateral chorioretinal coloboma associated with a macular neovascularization. Through this report, we intend to emphasize that despite its rarity, neovascularization remains a potential complication of chorioretinal coloboma. Furthermore, it underscores the importance of a thorough examination of Optical Coherence Tomography (OCT) in the accurate identification of neovascular membranes. The diagnosis was confirmed with fluorescein angiography and OCTangiography.
RésultatsMethods and results :
A 17-year-old girl diagnosed with chorioretinal coloboma presented with vision loss in the right eye. On OCT subretinal fluid was present. Looking through the different slabs on OCT a hyperreflective subretinal lesion was observed at the edge of the coloboma, accompanied by subretinal fluid extending towards the fovea. This characteristic OCT appearance was suggestive for a type 2 neovascular membrane. Subsequent fluorescein angiography revealed a well-demarcated hyperfluorescent lesion in early phase with late leakage. OCT angiography also confirmed the presence of a neovascular membrane in the outer retinal layer. An intravitreal injection with bevacizumab was performed, which led to an almost complete restoration of the patient’s previous visual acuity and resorption of the fluid on OCT with a small well-demarcated lesion remaining.
ConclusionThis case illustrates the rare association between chorioretinal coloboma and macular neovascularization. A thorough look at the OCT in combination with the typical image of a type 2 macular neovascularization on OCTangiography and fluorescein angiography will lead you towards a correct diagnosis.
