TitreAcute corneal haze after Selective Laser Trabeculoplasty : case report
ButSLT is a well-known procedure for patients with glaucoma and intraocular hypertension. While being a safe procedure, adverse effects may exist. The most common ones are transient and easily treated. However, rare and severe effects, such as corneal haze have also been reported. This case describes an acute corneal haze that persisted despite medical treatment following SLT.
MéthodesWe report the case of a 67-year-old man with high myopia and high IOP who underwent SLT. The procedure was performed without immediate complications. However, the control at 2 weeks afterwards highlighted a major stromal haze, being the cause of significant visual acuity reduction. Despite extended course of topical steroids and ciclosporine, the corneal haze showed minimal improvement over several weeks.
RésultatsAltough very rare, corneal haze after SLT has been described and is still not yet understood. Various mechanisms have been proposed: cytokine releases, herpes simplex reactivation or toxicity of residual alcohol on the goniolens but none of these theories provides a satisfactory explanation. Our review through the literature revealed a possible link between high myopia and this side effect. More reported cases are needed to draw a significant conclusion but, in the meanwhile, we believe that caution should be exercised when performing SLT on a highly myopic patient.
ConclusionPersistent corneal haze following SLT is an exceptionally rare but challenging complication. Standard treatments such as topical steroids may not always yield satisfactory results. A link with high myopia potentially exists. Therefore, we recommend being cautious with SLT in high myopic patients.
