TitreFine needle diathermy combined with intrastromal injection of bevacizumab to treat corneal neovascularization
ButTo report the outcome of a combined treatment of corneal neovascularization with fine needle diathermy and intrastromal injection of bevacizumab 2,5mg/0,1mL.
MéthodesWe treated four patients presenting corneal neovascularization with both fine needle diathermy and intrastromal injection of bevacizumab. Among them, two had suffered from HSV keratitis, one from HSV keratitis and perforation, and one from neurotrophic keratitis after brain surgery for a meningioma. The patients’ follow-up were analysed based on a slit lamp exam, best corrected visual acuity and the comparison of clinical pictures made before the surgery with ones made one month and three months after the surgery.
RésultatsFor half the patients, the blood flow was unnoticed in the cauterised new vessels after one and three months follow up.
For two patients we had to repeat the procedure, better targeting the new vessels, resulting in a better outcome.
We also noticed in one patient the regression of the lipid deposition after one month follow up.
Another patient presented a clearer cornea at one and three months, with less haze.
We were also able to gradually reduce the steroid drops in all the patients.
ConclusionA treatment with fine needle diathermy combined with intrastromal injection seems promising to limit corneal new vessels complications such as lipid deposition, and could help reduce the frequency of chronic eye drop instillation.
