Méthodes | Observational case report. |
Résultats | A 21-year-old Caucasian woman, on oral anticonceptives, presented with sudden-onset, bilateral, paracentral scotoma two days following her first intramuscular dose of the AstraZeneca Vaxzevria ChAdOx1 nCoV-19 (AZD1222) vaccine against SARS-CoV-2. Her ophthalmic and general history were unremarkable. Dilated fundoscopy showed faint, red brown perifoveal lesions in both eyes, which were particularly highlighted on near-infrared reflectance (NIR) imaging. On spectral-domain optical coherence tomography (SD-OCT), a disruption in the inner/outer segment (IS/OS) junction was visible in the region of the petaloid lesions, as well as a thickened outer plexiform layer. These features on multimodel imaging were consistent with acute macular neuroretinopathy (AMN). |
Conclusion | We describe a case of AMN occurring shortly after administration of the first first dose of the ChAdOx1 nCoV-19 vaccine. This association may indicate an immune-mediated pathway, however, the exact mechanism remains elusive. |
