TitreAsymptomatic bilateral choroidal metastases from endobronchial carcinoid.
Abstract Nr.P124
ButTo describe a patient with bilateral multifocal choroidal metastases 6 years after left pneumectomy for an endobronchial carcinoid.
MéthodesA 60-year old woman who presented with photopsias in the left eye underwent an extensive ophthalmological examination, including fluorescein angiography, OCT and ultrasound.
RésultatsBest-corrected visual acuity was 10/10 in both eyes. Slit-lamp examination was unremarkable. Fundoscopy revealed a small retinal tear in the left eye, for which she received laser treatment. In addition, choroidal masses were detected in both eyes. Ultrasound of these three distinct lesions showed moderate internal reflectivity.
Based on the medical history of a pneumectomy in 2007 for a pulmonary carcinoid and elevated chromogranin A blood levels, we concluded that these choroidal lesions are most likely metastases from the resected endobronchial carcinoid. Subsequently, a Gallium-68 scan proved other spinal cord metastases and mediastinal lymph node invasion.
Systemic treatment with somatostatin was started. Response to treatment is evaluated based on the level of plasma chromogranin A and ophthalmological follow-up.
ConclusionEndobronchial carcinoid tumors can metastase to the choroid. Because of the indolent nature of carcinoids, follow-up of patients, after resection of the primary tumour, is recommended for at least 20 years.
