REQUEST TO OPEN AN ACCOUNT
What should you do before requesting an account ? 

Before requesting an account on ophthalmologia.be, please check the list of required documents and information. This will help streamline the process and prevent delays caused by back-and-forth emails to gather missing details. Having everything ready in advance will save time and make account creation smoother.

First we need to know your occupation. Are you:
  1. Ophthalmologist, 
  2. Resident in Training in Ophthalmology, 
  3. Student (Medical Students, Orthoptist & Optometrist Students), 
  4. Orthoptist, 
  5. Optometrist, 
  6. Nurse, 
  7. Eyecare Personel.
Depending on your occupation you will need to supply us the following: 
Ophthalmologist: 
>>> First name + family name + your email address + RIZIV/INAMI  nr (0-00000-00-370)
Resident in Training Ophthalmology (1st, 2nd and 3rd Year): 
>>> First name + family name + your email address  + RIZIV/INAMI  nr (0-00000-00-037)*
*If you are first year resident and have not yet your Riziv nr, you need to supply a certificate that you have been accepted for Resident in Training of your institution and name of your mentor. 
Student (Medical Students, Orthoptist & Optometrist Students)
>>> First name + family name + your email address  + Name of your educational Institution
Orthoptist & Optometrist: 
>>> First name + family name + your email address  + RIZIV/INAMI nr & Visum Nr
Nurse:
>>> First name + family name + your email address  + Name of your workplace
Eyecare Technician:
>>> First name + family name + your email address  + Name of your workplace

WITHOUT THE ABOVE INFORMATION WE CAN NOT CREATE THE ACCOUNT !
So please when you email make sure to supply the requested information. 

1 person = 1 account, you can not register another person through your account. Each account has a unique email address. You can not have two accounts with the same email address.
