| Title | Comparative study of PRESERFLOTM MicroShunt, XEN45® Gel Stent and Trabeculectomy in open-angle glaucoma |
| Purpose | To compare the safety and efficacy of PRESERFLOTM MicroShunt (MS), trabeculectomy (Trab) and XEN45® Gel Stent (XEN) in patients with open-angle glaucoma during the first year. |
| Methods | A single-centre retrospective case-control study was conducted on all eyes that underwent standalone glaucoma surgery between 2013 and 2020. The primary outcome was surgical success after 1 year. Complete success was defined as intraocular pressure (IOP) of 6–15 mmHg or 6–18 mmHg, without vision loss, additional glaucoma surgery, or IOP-lowering medication. Qualified success allowed medication and/or laser therapy. Secondary outcomes included visual acuity, medication use, and complications. |
| Results | In total, 384 eyes of 384 patients were included, 82 eyes received a MS, 117 a Trab and 185 a XEN. MS achieved the highest rates of both complete and qualified success across both IOP criteria. Regarding complete success, MS was equal to Trab (p=0.207, IOP 6-15mmHg and p=0.230, IOP 6-18). MS (p<0,001) and Trab (p=0.030 and p=0.018, respectively) performed significantly better than XEN. For qualified success, MS showed superior outcomes, with statistical significance only observed comparing MS to XEN in the IOP 6-18 mmHg group (p=0.049). Significant hypotony was rare across groups and peaked in the first week. Minor interventions were more frequent after Trab. |
| Conclusion | MS and Trab achieved comparable success rates and outperformed XEN without use of medication. MS demonstrated superior efficacy over XEN regardless of medication use. MS and Xen need fewer post-operative interventions compared to Trab. |
