TitreEpithelial inoculation after SMILE: a case series
ButWe present a case series of epithelial ingrowth following small incision lenticule extraction (SMILE). In this case series we differentiate epithelial ingrowth from epithelial inoculation and discuss differential diagnostics and possible treatment strategies. Furthemore, we present a new classification to promote correct labelling and aid treatment planning.
MéthodesWe retrospectively reviewed 5 patients who developed epithelial ingrowth after SMILE. Clinical presentation, morphological features and tomography were reviewed. Treatment modalities, ranging from observation to surgical intervention, were used according to severity, possible cap melt and impact on visual function.
RésultatsThe 5 cases demonstrate variable manifestations of epithelial ingrowth, from localized, non-progressive lesions to visually significant and progressive involvement. Distinction between ingrowth and inoculation is essential, as the latter often remained stable without intervention, while progressive ingrowth may require prompt management. Several cases demonstrate stable ingrowth/inoculation, even without surgical intervention.
ConclusionThe current case series highlights the wide variability in the evolution and management of epithelial inoculation or ingrowth after KLEx/SMILE. We believe the true incidence of epithelial ingrowth/inoculation is probably higher than assumed, presumably due to underreporting. Therefore, we propose a first initiative for a new classification for epithelial ingrowth and inoculation after KLEx surgery to promote correct labelling, further research and enhancement of management strategies.
