TitelImplantable Phakic Contact Lens (IPCL): A Systematic Review
DoelTo assess the visual outcome and safety of the (diffractive) Implantable Phakic Contact Lens (IPCL). Comparative data to the Implantable Collamer Lens (ICL) will be provided when possible. The IPCL is a new posterior chamber phakic intra-ocular lens (pIOL) which is less expensive, has more sizing options and is injected through a smaller corneal incision. For presbyopia correction, the diffractive IPCL is currently the only pIOL available.
MethodesA systematic literature search was performed and lectures on international conferences were screened for additional data. Only original studies were considered.
ResultatenThis review includes 25 articles and 2 lectures. IPCL implantation showed a reliable visual outcome with an efficacy index of 1,06. The average post-operative UDVA (0,22 logMAR) is worse compared to the ICL (UDVA -0,01 logMAR) but can be attributed mainly to the suboptimal average pre-operative CDVA (0,23 logMAR). Diffractive IPCL implantation in a presbyopic population (average 48 years) proved to have a good visual acuity for both far (UDVA 0,06 logMAR) and near (UNVA 0,04 logMAR) with little visual side effects.
IPCL implantation shows to be safe with a safety index of 1,23 and a postoperative CDVA (0,11 logMAR) clearly higher than the pre-operative CDVA. Cataract was found in 0,67% of IPCL V2.0 cases and endothelial cell loss was found to be 3,1% 12 months after surgery. No cases of acute glaucoma or retinal detachment after IPCL V2.0 implantation were described.
ConclusieThe (diffractive) IPCL is a promising new pIOL. Current data shows overall good results but interpretation is hampered by the suboptimal pre-operative CDVA leading to worse UDVA postoperative compared to ICL. Further research is needed to draw a more definitive conclusion.
