TitelReal-world effects on intraocular pressure of combined Micropulse transscleral laser therapy with phacoemulsification
DoelTo report outcomes on intraocular pressure (IOP) of combined treatment composed by Micropulse transcleal laser therapy (TLT)and phacoemulsification in patients with coexisting cataracts and glaucoma.
MethodesThis retrospective case series of 37 eyes of 33 patients was conducted at Citadelle Hospital in Liege, Belgium between 2021 and 2024. These patients underwent MicroPulse TLT and cataract surgery during the same setting under general anesthesia by two operators. The protocol of TLT was standardized to deliver a power of 2500 mW for 80 seconds for every 180-degree arc with 31,3% duty cycle.
Pre operative parameters were IOP assessed by Goldmann applanation tonometer (GAT) and numbers of glaucoma medications and were compared with these at 1 month, 3 months and 6 months.
Resultaten37 eyes of 33 patients (48% were female) underwent combined surgery. The mean age was 69,7 years. Seven eyes had cataract and pseudo-exfoliation, 18 eyes primary open-angle glaucoma, 12 eyes were chronic angle-closure glaucoma. The mean baseline IOP was 18,72 mmHg with an average of 2,8 medications. At 1 month, IOP was decreased at 15,67 mmHg, 14,68 at 3 months and 14,83 at 6 months postop with no significative decreased of numbers of glaucoma medications (2,6).
ConclusieCombined surgery composed by Micropulse TLT and cataract surgery can reduced IOP in glaucomatous patients with significant cataract. Adding Micropulse TLT to phacoemulsification could prevent late postoperative corticoresponse in glaucomatous patients.
