TitelBartonella henselae-related conjunctival granuloma – case report and literature review
DoelWe report a case of Bartonella henselae-related conjunctival granuloma and put it into perspective upon literature review.
MethodesA 6-year-old girl was referred to our center with a red, itchy conjunctival lesion in the right eye, right preauricular and cervical lymphadenopathies, and a right parotid swelling.
ResultatenThe visual acuity was 20/20 in both eyes. The examination of the right eye showed some conjunctival follicles and a reddish fleshy conjunctival lesion emerging from the semilunar fold and descending to the inferior fornix.
The patient underwent a complete pediatric evaluation with a blood test. The ultrasound of the face showed right intraparotid and submandibular lymphadenopathies. Head MRI excluded any other intra-orbital or intracranial lesions.
The blood test revealed highly positive Bartonella Henselae IgG and IgM, very suggestive of a recent infection; while the conjunctival lesion was already spontaneously decreasing and disappeared completely after 4 weeks; the lymphadenopathies resolved after 8 weeks.
Because of the spontaneously favorable evolution, no systemic nor local treatment was given.
ConclusieAlthough an association between unilateral granulomatous follicular conjunctivitis and ipsilateral regional lymphadenopathy is well known (Parinaud oculoglandular syndrome, mostly due to Bartonella), it is exceptional to see a bulbar conjunctival lesion of that extent. Upon literature review, we could only find two cases of a similar but smaller lesion, arousing from the conjunctiva.
We feel it is important for the clinicians to be aware of the possible ocular conjunctival manifestation of Bartonella henselae in form of a large granuloma, which can facilitate timely diagnosis avoiding unnecessary treatment or interventions.
