TitelDescemet stripping automated endothelial keratoplasty : A 10-year retrospective study at Erasme Hospital
DoelPenetrant keratoplasty has long been the treatment of choice for corneal endothelial dysfunction. However, it has been gradually replaced by endothelial grafts due to a faster visual recovery with fewer postoperative complications. In this context, we performed a 10-year retrospective study at Erasme Hospital on DSAEK endothelial grafts to evaluate the visual and refractive results as well as the efficacy and safety of this technique.
MethodesA retrospective study was carried out at Erasme Hospital on 223 eyes included among 300 having benefited from a DSAEK corneal endothelial graft associated or not with phacoemulsification between January 2011 and December 2020. The primary outcome was best corrected visual acuity (BCVA). Secondary outcomes were spherical equivalent and postoperative complications.
ResultatenA total of 223 eyes of 176 patients (mean age 71 years) were included (68% with Fuchs dystrophy and 17% with pseudophakic bullous keratopathy). The mean best corrected visual acuity expressed in log(MAR) decreased from 0.78 (1.6/10) to 0.32 (5/10) at 12 months. We noted an overall mean spherical equivalent change at 12 months of + 88 D, + 1.119 D in the DSAEK group and + 0.652 D in the DSAEK + Phacoemulsification group. The main postoperative complications were graft detachment (10%) and graft failure (4%).
ConclusieDSAEK is a safe and reliable surgical procedure for the management of corneal endothelial diseases and offers rapid visual recovery with good short- and medium-term results.
