Doel | We describe a case of a 42-year-old female patient presenting a bilateral Purtscher-like retinopathy after a viral infection. |

Resultaten | A 42-year-old female patient was referred to our department with bilateral signs of retinal ischemia. She complained about an acute visual deterioration on both eyes accompanied by flu-like symptoms. Best-corrected visual acuity (BCVA, decimal) was 0.4 and 0.3 in the right and left eye, respectively. Ocular fundus examination, optical coherence tomography as well as fluorescein angiography revealed bilateral peripapillary cotton-wool spots with superficial haemorrhages, intraretinal whitening, along with a cystoid macular edema. Systemic evaluation including neuroimaging, abdominal CT as well as an extensive bloodwork, revealed no abnormalities, other than signs of a recent viral infection with elevated lymphocytosis. She was diagnosed with bilateral Purtscher-like retinopathy and was treated with systemic corticosteroids. Three months later fundus examination and optical coherence tomography showed regression of most cotton-wool spots with a complete resolution of cystoid macular edema, but clear disorganization of inner retinal layers with focal disruptions of the macular ellipsoid zone in the right eye. BCVA was 0.5 and 0.6 in the right and left eye, respectively. |
Conclusie | Purtscher-like retinopathy is a rare vasooclusive retinopathy, which can occasionally occur after a benign viral affection. Clinical course and visual improvement vary, depending on the integrity of the various retinal layers. |
