DoelTo report a case of episcleritis in hypocomplementemic urticarial vasculitis syndrome (HUVS).
MethodesA 56-year-old Caucasian woman presented with bilateral episcleritis. She underwent a complete ophthalmological and systemic work-up.
ResultatenShe presented with episodes of bilateral redness since more than 20 years ago. She had a history of skin lesions, arthritis and glomerulonephritis. Biomicroscopy showed bilateral episcleritis. In 1981 she reported a first episode of non-pruritic, urticarial wheals on both hands, migrating to the whole body, but sparing the face. Biopsy showed neutrophilic dermatitis without necrotising vasculitis. She developed episodes of arthritis, angioedema, abdominal pain and glomerulonephritis. Renal biopsy showed proliferative glomerulonephritis with mesangial deposition of C3 and C1q and subintimal deposition of C3, C1q and IgM. Laboratory results showed reduced C3, CH50 and C4. C1q complexes were positive. ANA, anti-dsDNA, anti-ENA and RF were negative. ESR was elevated. CBC renal and hepatic function were within normal limits. Protein analysis was normal. A diagnosis of HUVS was made on the basis of two major and five minor criteria.
ConclusieEpiscleritis, conjunctivitis and rarely uveitis may be observed in HUVS. Although HUVS is relatively rare, it should be included in the differential diagnosis of these conditions. Awareness of HUVS and rational diagnostic evaluation will prevent misdiagnosis.
