Doel | To determine the role of Optic Nerve Sheath Diameter (ONSD) measurement by Ultrasound (US) in the context of possible raised intracranial pressure (ICP) in children. |
Methodes | Retrospective case series review in a large university medical center in the Netherlands. A large US-ONSD database was manually searched for cases (age range 0-16yrs) in which the aspect of the optic disc raised suspicion of oedema and for whom an US-ONSD measurement was requested. Paediatric case notes were scrutinized for relevant, and esp. missed, diagnoses with concomitant raised ICP after the ONSD measurements. |
Resultaten | Out of 93 cases 46 had an ONSD of 5 mm or more. 9 of these had syndromes with raised ICP. Out of the remaining patients with an ONSD of less than 5 mm 1 was diagnosed with raised ICP. The negative predictive value for raised ICP with an ONSD of less than 5 mm or less was 98%. |
Conclusie | US-ONSD measurement appears a useful tool to rule out, rather than confirm, raised ICP in children with suspected optic disc oedema. |
