DoelTo evaluate the safety and efficacy of iodine 125 plaque brachytherapy (IBT) for selected large uveal melanomas.
MethodesThis prospective nonrandomized study included all patients referred to our institution and diagnosed with uveal melanoma with a thickness > 8.0 mm. The eligibility criteria included patients who declined enucleation and/or were not eligible for tumor resection (poor health status, retro-equatorial tumor, largest basal tumor diameter > 15 mm) and a follow up of at least 24 months.
ResultatenThirteen eligible patients with a mean age of 61 years old underwent IBT (mean dose to tumor apex, 90 Gy). Mean tumor height was 9.3 mm and largest basal tumor diameter was 15.3 mm. After a mean follow-up of 48 months, no case of local tumor recurrence or late enucleation was recorded. Treatment related complications included cataract (n=8, 61%), retinopathy (n=8, 61%), neovascular glaucoma (n=1, 8%), scleral necrosis (n=1, 8%), and vitreous hemorrhage that required vitrectomy (n=4, 30%). Final vision worse than 20/200 was documented in 8 patients ( 61%). Metastasis developed in 4 patients (31%) after a mean interval of 19 months.
ConclusieAlthough the long-term prognosis of preserving vision is poor, IBT seems to be a safe and effective alternative to enucleation with regard to local tumor control. Benefit from enucleation as regards avoiding IBT-related complications is smallest for patients in whom the risk for metastasis is high.
