DoelTo evaluate the long-term survival after enucleation for uveal melanoma and to detect the main histopathological prognostic factors.
MethodesBetween 1973 and 2003, a total of 600 patients who were enucleated for uveal melanoma, were included in this study. Histopathological data on diameter and height, the presence of a retinal detachment, necrosis, pigmentation, mitotic rate, vascular ingrowth, scleral ingrowth, PAS positive patterns, integrity of Bruch's membrane, and cell type were collected.
ResultatenThe cumulative 5-, 10-, and 15-year survival for melanoma-related death was 72.2 % (SD 2%), 63.1% (SD 2%), and 58.1% (SD 2%), respectively. Most significant prognostic factors were the presence of Loop PAS-patterns (P<0.001; HR 5.448) and largest basal tumor diameter (P<0.001; HR 2.139) in univariate Cox regression analysis. In a multivariate model, Loop PAS-patterns (P<0.001; HR 6.454) and basal tumor diameter (P<0.001; HR 1.248) were again the most significant factors. Only integrity of Bruch's membrane and cell type could improve the multivariate model.
ConclusieLoop PAS-patterns were the most important prognostic factor in uveal melanoma. Largest basal tumor diameter, cell type and invasion of Bruch's membrane were independent significant factors for survival after enucleation in uveal melanoma.
