TitleComplete regrowth of the iris pigmentary epithelium after trabeculectomy
PurposeTo present the case of a 63 years-old woman who developed complete regrowth of the iris pigmentary epithelium (IPE) after trabeculectomy (TE).
MethodsCase report (with supporting pictures).
ResultsThe patient underwent TE for secondary glaucoma, the surgery and postoperative course being unremarkable. However, four months later, she presented with a flat bleb, an intraocular pressure (IOP) of 50 mmHg and anterior chamber (AC) inflammation. We observed IPE regrowth, obstructing the iridectomy and the scleral flap opening. An AC tap and a blood sample were collected, and YAG-laser performed on the new tissue. We prescribed anti-inflammatory drops and eye massages. Six months after TE, the iridectomy opening and the scleral flap aperture were clear, the AC quiet and the IOP low without medication. Both AC tap and blood analysis were unconclusive.
ConclusionIris regrowth has already been described after laser periferal iridotomy. Its causes are unknown, although intraocular inflammation seems to be crucial. However, to our knowledge, there is no previous reported case of IPE regrowth after TE.
